New Customer Account Enquiry Form
Thank you for your interest in setting up an account with Oxford Pharmacy Store, please fill out the enquiry form below with the relevant information and our Customer Services Team will send you the New Customer Verification Form to fill out.
Important details for returning the New Customer Verification Forms to OPS.
Note: Failure to complete the following sections on the New Customer Verification forms will result in a delay to opening your account:
- Customer Details and Customer Verification sections
- Supply of Unlicensed Medicinal Confirmation Form
- OPS cannot accept typed name signatures, please make sure the documents either have an electronic signature or are wet signed.
Please return the signed forms along with a Purchase Order (PO) to ops.orders@oxfordhealth.nhs.uk, without the forms and PO we will be unable to set up your account. Should you need any information on products prior to raising a PO please do not hesitate to get in contact via the Quick Contact Form.
Once your account has been approved your order will be processed and going forward you will be able to purchase via email, GHX or our website. You will be set up a website login and will be able to view our full catalogue online.
